They include the Bayshore Health Care & Rehabilitation Center, a nursing home, The Willows At Holmdel, an assisted living community, an Ambulatory Surgery Center in Old Bridge, a SleepCare Center in Holmdel, a SleepCare Center in Freehold, and two Fitness & Wellness Centers, one in Hazlet and one in Old Bridge.
